You get this hat by gaining Friendly reputation with the Bloodsail Buccaneers. You have to kill Steamwheedle Cartel goblins in order to do this. Once you get Friendly, you have to do a simple quest to kill two npcs in Booty Bay. You also can get Bloodsail clothing for being Neutral with them.
